Plant accident kills = worker

State labor investigators are reviewing the death of a Yakima man who = fell=20 from a conveyor belt that was mistakenly activated while he was working = on it, a=20 relative and state officials said Saturday.

The incident that killed 62-year-old Roland Couch happened Monday = morning at=20 the Yakima Resources plant, 617 N. Eighth Street, his sister Vicki = Griffin said=20 Saturday in a telephone interview from East Wenatchee.

The Yakima Fire Department responded to an aid call at that address = at 7:43=20 a.m. Monday. City firefighters said they could not release information = about the=20 patient's medical treatment.

According to the account that Griffin received, Couch sustained = massive=20 injuries after being propelled off the belt an estimated 25 feet and = landing on=20 asphalt.

He died near midnight Wednesday at Harborview Medical Center in = Seattle,=20 Griffin said. A Harborview nursing supervisor confirmed the death = Saturday.

David Puente, Yakima compliance manager for the state Department of = Labor and=20 Industries' workplace safety division, said the agency is investigating. = He=20 could not elaborate on the details of the incident, but said he received = an=20 initial report that roughly matched the description given by Griffin. =

Griffin said she had been told that her brother was conducting a = routine=20 lubrication drill atop the conveyor, used either to move logs or other = material,=20 when a co-worker mistakenly activated the equipment. Couch normally = would have=20 been finished applying the lubricant by the time the co-worker came on = duty,=20 Griffin said.

Couch had been working at Yakima Resources since last fall.

Puente was not immediately able to confirm the name of Couch's = employer, but=20 he said it was believed to be a contractor for Yakima Resources, which = is owned=20 by Frontier Resources.

Griffin said her brother's employer was from Pilot Rock, Ore., where = Frontier=20 subsidiary Kinzua Resources has offices.

A message left Saturday night at the home of Jeff Demers, a Frontier = vice=20 president whose name is also listed as a Kinzua contact, was not = immediately=20 returned.

Yakima Resources in January applied for a city permit to demolish = buildings=20 associated with the plant's closed sawmill. It was not clear whether = Couch was=20 working on that project.

Yakima Resources took over the old Boise Cacade plant, made up of = both the=20 sawmill and a plywood plant, in 2004. The sawmill closed in August.
